About this SIG

Models of Care

The SIG/Working Group brings together clinicians to collaborate on design, implementation and evaluation of General Medicine models of care, and to share learnings across varied clinical settings (metro, regional, rural and remote) in Australia and New Zealand.

Objectives
  1. Provide an accessible forum and high-level input to discussions on General Medicine models of care and workforce issues across Australia and New Zealand.
  2. Enable regular sharing of information, mutual learning, and collaboration on development, evaluation and reporting of models of care, embracing heterogeneity and diversity.
  3. Build understanding of current models of care and their effectiveness across the patient journey (community/ambulatory care, ED interface, HITH, inpatient, intermediate care) and settings (metro, regional, rural, remote).
  4. Describe drivers behind the evolution of models of care (e.g. patient complexity, ED access, bed pressures, budget constraints).
  5. Collaborate with broader communities of practice (e.g. pharmacists, nurses, allied health) that contribute to effective models of care.
  6. Develop measurable performance and quality indicators relevant to General Medicine.
  7. Provide high-level guidance on optimal staffing/workforce to operationalise models of care in diverse settings.
  8. Develop standardised guidelines for development, evaluation and reporting to improve generalisability and shared learning.
